Hopefully, the power wire is damaged internally from "wear and tear". Avoid this damage by avoiding any tension, kinking, etc. on the wire. You should get a new power adapter, or get it repaired. After a long time, the battery will start to lose functionality, also.
The other thing that could be wrong is your power port on your laptop is damaged. That would need to be repaired. Again, hopefully, it is your wire that is the problem.
